The California State Health Department have several forms that are not available in electronic format, however we are pleased to send it to you by mail.

Please make sure to enter the correct necessary information in the email you will send to us. Also take a moment to read through the Frequently Asked Questions and Manual of Procedures Sections to see what is the correct form you will need to request.
On your request, you MUST enter the following information:
- Your complete name;
- Complete address where the material should be mail to (don't forget the zip code);
- What type of Form you are requesting;
- Reason for requesting the form (this will be used in case you are requesting the wrong form for the wrong purpose)
