Access to Health Care & Social Services

2-1-1 Information and Referral Phone Line helps people find affordable housing and critical social services in Berkeley and in Alameda County.

Affordable Care Act (ACA) also known as Health Care Reform or Obamacare, means more people will have access to affordable health insurance.  In addition, all new health plans are being required to provide preventive services like mammograms and immunizations without charging a deductible, co-pay or coinsurance. 

MPj04327020000[2].jpg(2)Child Health & Disability Prevention Program (CHDP)  is a health promotion and disease prevention program serving income-eligible children.  Free periodic health assessments are available to low-income children through the CHDP Program.  Assistance getting treatment services and low-cost insurance for infants, children, and youth is also available.

El Centro provides free assistance with Medi-Cal/Healthy Families applications, and referral services to all, including the Spanish speaking community of Berkeley.

Ann Chandler Public Health Center provides comprehensive reproductive health care, family planning services, immunizations, tuberculosis skin testing, health education, and referrals to thousands of diverse community members each year.  Many of the services are subsidized.

Berkeley High School Health Center offers free and confidential medical, health education, and mental health services to all students at Berkeley High School, students of Independent Studies, and Berkeley Technology Academy.

Birth & Death Certificates

The City of Berkeley Office of Vital Statistics has maintained documentation of births and deaths that occurred within Berkeley City limits since 1895.  If a birth or death occurred outside Berkeley, contact the county, state, or country in which the event occurred to request the record. 

Communicable Disease Reporting 

The Communicable Disease Control Program works in partnership with the medical and neighborhood communities to control and prevent the occurrence and spread of communicable diseases through prevention, surveillance and outbreak control.

Meaningful Use Requirements

Meaningful Use Requirements 
refer to the Meaningful Use provisions of the Health Information Technology for Economic and Clinical Health Act were enacted as part of the American Recovery and Reinvestment Act of 2009.  Under this Act, Eligible Hospitals (EH), Critical Access Hospitals (CAH), and Eligible Professionals (EP) that provide clinical services to Medicare and Medicaid patients and commence transmission of electronic health data are eligible to receive Meaninful Use financial incentives from the Centes of Medicare and Medicaid Services.
